We recommend you use the following steps to become a member of SBCMS:

1. Download the Application Form.  (The application is currently only available in MS Word format)

2. Open the file and print it out.

3. Complete the form, making sure to place your signature in the given space at the bottom.

4. Either fax or mail the form to SBCMS.

If you are unable to open word documents, contact SBCMS to receive the application form in the mail.
