presented to

Fred W. Flotho

This twenty sixth-day of June, in the year two thousand three

Fred Flotho has been an advocate for persons living with HIV/AIDS since 1987. He has served tirelessly on a number of local advisory committees including the San Bernardino County AIDS Advisory Taskforce (1987-90), San Bernardino County CARE Consortium (1990-95), San Bernardino County HIV Prevention Community Planning group (1991-current), Riverside/San Bernardino CA Housing Committee (1993-current), and the Inland Empire HIV Planning Council (1993-current). He has also been appointed to three committees at the state level. These include the Ryan White CARE Act Title II Working Group (1994-99), Community Planning Working Group (1994-2000) and the California HIV Planning Group (1996-current).

Mr. Flotho has served as a leader as well as a contributing member. He has been appointed as co-chair of the Inland Empire HIV Planning Council where he chairs the Membership Committee and is Co-chair of the Planning and Evaluation Committee. Fred also serves as a member of the council's Executive and ad hoc Food Services committees. This level of involvement is not only time intensive, it is labor intensive. Fred has been a regular contributor to all aspects of planning for the delivery of ambulatory/outpatient medical care and support services which include case management, substance abuse counseling and treatment, transportation, home health care, food services, mental health counseling, dental care and housing assistance, to persons living with HIV/AIDS.

This gentleman has truly made a difference in planning, implementation and evaluation of HID/AIDS-related services within San Bernardino County and the state of California. His number of hours of community service is only exceeded by the quality of his work and the depth of his concern for persons living with HIV/AIDS. It is an honor to present this 2003 SBCMS Outstanding Citizen Award to Fred W. Flotho.

J. Frank Randolph, M.D.
President
