Healthcare Provider Survey

In March, I discussed the medical society and its African American Health Initiative's (AAHI) countywide health-planning project. A part of the planning process is to identify all current prevention programs and treatment services for five (5) major health conditions (i.e., heart disease, high blood pressure, HIV/AID, breast and prostate cancer) that focus specifically on the Black/African American population in San Bernardino county. The following Healthcare Provider Survey is the assessment tool used to determine services provided for the a-for-mentioned conditions among this population. To complete this survey please go online to http://www.aahi.info/providersurvey you will find this instrument under the African American Health Planning Project link. Please have the appropriate person(s) in your practice to complete this electronic survey. The information contained in the survey is critical, please follow the instructions given and answer all questions. Staff from the AAHI Planning Project will contact your office to assist your staff in completing this survey. Thank you for your time, interest, and support. We believe, together we all can make a difference in providing quality healthcare for all residents in San Bernardino County.
